Language and setting

The verb נִשְׁמַמְתִּי (nishmamti—'I am desolate/numb') suggests not mere numbness but existential desolation; נִשְׁבַּרְתִּי (nishabarti—'I am broken') employs the language of something completely fractured beyond easy repair. The groan (נָהַמְתִּי, nahamti) is not articulate speech but the inarticulate cry of profound distress.

The psalmist in the nadir of his suffering, when pain becomes so complete that coherent speech gives way to animal outcry.

Significance
