David is describing what we might call psychosomatic illness—where spiritual and emotional turmoil manifests in real physical ways. He's not separating body from spirit; they're one integrated reality. In his understanding, his sin has thrown his entire being out of order.

This reflects the biblical understanding that sin doesn't just affect your standing with God—it affects your whole person. When things are wrong spiritually, everything else starts to unravel.
The psalmist correlates spiritual guilt with physical illness, asserting that his bodily disintegration—lack of soundness and rest in his bones—stems directly from God's anger at his sin.
The Hebrew worldview did not bifurcate body and soul as later Hellenistic philosophy did; rather, the person was an integrated whole, so sin manifested somatically. The bone (עֶצֶם, etzem) represented the innermost structural integrity of one's being, so its disease signified fundamental breakdown of personhood.
The psalmist, articulating a theodicy that accepts suffering as deserved consequence rather than arbitrary affliction—a perspective that distinguishes biblical lament from mere complaint.
This reflects the Deuteronomic theology of covenant curses (Deuteronomy 28) while also prefiguring the problem of suffering that Job and later wisdom literature would interrogate more deeply; ultimately, Christ's vicarious suffering breaks the mechanical equation between sin and sickness.
