Everyone you counted on—your loved ones and your family—have abandoned you because of your illness, leaving you completely isolated.
In David's world, illness could be contagious or spiritually dangerous, so people would distance themselves. But beyond that, when you're suffering and struggling, many people simply don't know what to do with that, so they disappear. David's experiencing both physical illness and social death.
David, naming the crushing loneliness of abandonment
You'll see this pattern in Scripture: people in deep trouble often face isolation on top of their other suffering. Job's friends eventually abandoned him; Jesus's disciples fled when He was arrested. But ultimately, God never abandons.
The psalmist finds himself socially isolated—even his loved ones and closest relations withdraw from him, apparently due to contagion-fear or revulsion at his diseased state.
The Hebrew דּוֹדִי (dodi—'my beloved') and קְרוֹבַי (qerovai—'my kinsmen') represent the innermost circles of attachment; their withdrawal compounds the physiological suffering with profound loneliness. This reflects ancient Mediterranean practices of shunning the ritually or medically impure, though it also hints at moral judgment—neighbors interpreting his illness as divine punishment.
The psalmist, now articulating the secondary suffering of abandonment that frequently accompanies visible illness or social disgrace.
This parallels Job's experience with his friends (Job 19:13-14) and prefigures Jesus' abandonment at Gethsemane and Golgotha—though the Incarnate Son's rejection becomes redemptive in ways the psalmist cannot yet comprehend.
