Jacob formally calls his sons to attention, reminding them he's their father and they need to really listen to what he's about to say.
This is Jacob emphasizing the weight of the moment. He's using both his names—'Jacob' (his birth name) and 'Israel' (his covenant name given by God)—to show this isn't just a dying man rambling. This is Israel speaking, the man God renamed and through whom God's promises flow.
