Seven healthy, well-fed cows came up from the Nile and grazed among the reeds. Pharaoh repeats the image of abundance and prosperity from his dream.
Pharaoh is being as precise as he can be, because he wants Joseph to understand exactly what he saw. The healthy cows matter—they represent plenty, and he wants to make sure Joseph grasps that contrast with what comes next.

The repetition of this image—both times Pharaoh sees it and now as he tells Joseph—emphasizes its importance. God wants Joseph to see clearly what's coming.

19After them, seven other cows—sickly, ugly, and thin—came up. I have never seen such ugly cows in all the land of Egypt!
20Then the thin, ugly cows devoured the seven well-fed cows that were there first.
